How should you prepare for your first appointment?

To help your orthodontist better address your concerns and answer your questions on the day of your first appointment, please be sure to fill out the first-visit questionnaire on our DENTAPOCHE app. You should have received your login credentials and password via email in advance to complete this questionnaire.

We may have to reschedule your appointment if we do not receive the questionnaire as expected before your appointment.

The DENTAPOCHE app allows you to :

Access clinic information and your patient record (documents, photos, X-rays, etc.)

Receive notifications about your appointments.

Contact the clinic via instant messaging.

View your payment history, upcoming due dates, and access your upcoming invoices.

Experience has shown that patients prefer to answer these questions at home, taking the time to think them through.

Once completed, this questionnaire will be sent electronically directly to our front desk.
